1. The DD Form 93 is one of two emergency documents completed by every Servicemember. It is used to designate death gratuity (DG) beneficiaries in the event of death. Additionally, the form serves as a guide for the disposition of the Soldier's pay and allowances if captured, missing or confined. It is also used to identify the name of and address of the person(s) to be notified in case of emergency or death.
 
2. The SGLI-8286 (Servicemembers' Group Life Insurance Election and Certificate) is the second of the two emergency documents completed by every Soldier. While the DD Form 93 allows the Soldier to identify the person or persons who will receive death gratuity benefits, the SGLI-8286 is used to identify who will receive the proceeds of the government sponsored life insurance.
 
3. Over the last two years there have been a number of changes designed to provide Soldiers greater flexibility in determining who they designate to receive their benefits. Effective July 2008, a change to the existing law allows Soldiers to designate up to ten beneficiaries to receive the $100,000 (100K) DG benefit, in $10,000 increments; none of whom are required to be a spouse or Family member. In cases where the Soldier is married and elects the spouse to receive less than 100% of the DG, the Army will notify the spouse in writing.
 
4. The Department of the Army has directed that all Soldiers (Active, Guard or Reserve) who completed the DD Form 93 on either version of the form on or after 1 July 2008 must redo the DD Form 93 on the January 08 version as soon as conditions permit (METT-T). However, because of the new flexibility allowed to Soldiers, Commanders/HR Professionals should provide their remaining Soldiers an informed opportunity to review their DD Form 93 to ensure that it accurately reflects their wishes.
 
5. According to the new law, when a Soldier is married and the spouse is not the sole beneficiary for the DG benefits, the Army will notify the spouse in writing. This action is taken to ensure that the spouse, who in many cases will have the responsibility for burial, is aware of the disposition of DG benefits. Soldiers are encouraged to speak to the spouse prior to making this election or as soon as possible after the form are signed.
 

6. There are several options for completing the form, none of the preferred options require an HR Professionals be present, but all require an HR Professional to review the form. However, in all cases, where the Soldier has questions, an HR Professional should be consulted prior to signing the DD Form 93. Commanders may implement internal initiatives to meet this requirement or direct Soldiers to use one of the options below:

Option 1. To use AKO MyForms, go to the following URL: https://myforms.us.army.mil/wps/myportal. Once the page has loaded, enter DD 93 into the "Form Number" box. When the results screen appear, click "Record of Emergency Data." Complete the form, digitally sign it and route it to the HR Professional using the routing/tracking feature in AKO MyForms (contact your servicing S1/HR Professional for the appropriate POC). The HR Professional will review the form, execute the spousal notification if necessary, and forward the completed DD Form 93 to the Soldier's iPERM record (No witness is required for forms that are digitally signed). Option 2. Go directly to the form included on the OSD web site, go to the following URL: http://www.cpms.osd.mil/disasters/docs/DDForm93.pdf. The form can also be found on the U.S. Army's publication site www.usapa.army.mil. This version of the form allows the Soldier to fill out the form and digitally sign, it does not allow the Soldier to forward it directly to the HR Professional. After completing the form and digitally signing, the Soldier should print out the form and take it to the HR Professional. The HR Professional will review the form, execute the spousal notification if necessary, and forward the completed DD Form 93 to the Soldier's iPERM record (No witness is required for forms that are digitally signed). Option 3. When a Soldier uses a non-CAC (common access card or ID card as it is commonly known) enabled computer, once the form is complete, print the completed form, sign the completed form in front of a witness who also signs the form as the witness. Provide a copy to the unit's HR Professional for review. The HR Professional will review the form, execute the spousal notification if necessary, and forward the completed DD Form 93 to the Soldier's iPERM record (No witness is required for forms that are digitally signed). Option 4. Commanders/HR Leaders who have Soldiers in locations that lack computer support or who are otherwise unable to complete a new DD Form 93 on a computer may complete the form in hardcopy using pen and ink when METT-T conditions permit. In these instances, Soldiers must sign the completed form in front of a witness, who then signs the form as the witness. A copy is provided to the units HR Professional for review. The HR Professional will review the form for accuracy, execute the spousal notification if necessary, and forward the completed DD Form 93 to the Soldier's iPERM record when METT-T conditions permit. 7. Because the new changes have not been loaded into eMILPO, effective immediately, HR Professionals will stop using eMILPO to complete the DD Form 93. Any Soldier who used eMILPO to update their DD Form 93 on or after 1 July 08 must contact their HR professional to immediately schedule an appointment to redo the form using the January 08 version. All other Soldiers who used eMILPO or other means prior to 1 July 08 should be given an informed opportunity to review and update their DD Form 93.
 
8. If you use a computer to complete the DD Form 93, after completing the form and digitally signing with your CAC, forward to your HR Professional for review if using AKO MyForms or print it out and deliver to your HR Professional for review using the other options. Always consult a HR Professional if there are any questions pertaining to the completion of the form or the process. (See question six above.)
 
9. The accuracy of the DD Form 93 is essential to honoring the intent of the Soldier in the event of death. Inaccurate or outdated information on the form may result in the improper distribution of the $100K DG benefits.
 
10. The changes to the DD Form 93 apply to all Soldiers (Active, Guard and Reserve) regardless of where they are stationed. Commanders in a deployed location must engage their Rear Detachment Commanders immediately to take appropriate action to identify and assist Soldiers affected by these changes. Commanders of units preparing to deploy must ensure that all Soldiers deploying to the CENTCOM AOR have completed the January 08 version of the DD Form 93. Additionally, Family Readiness Groups and other communication means should be used to inform and educate Spouses and Families of theses changes.
